GREEN BAY - The Program Case Manager will be responsible for case management duties, and delivery of House of Hope programs and services to Clients in the House of Hope Emergency Shelter and Housing Stability programs. The Program Case Manager will also provide supervision and direction for Program Specialists regarding programs and facility maintenance.

Hours:

Part-time position. Ability to work flexible hours is necessary. Primarily afternoon and evening shifts. Some weekends and holidays required.

Essential Duties:

Provides case management activities, case meetings, and case plan oversight to Clients as directed by the Lead Case Manager. Supervises Program Specialists to ensure effective planning, implementation, and evaluation of direct Client services that will lead to high quality opportunities for the advancement and success of young mothers and their children in the areas of parenting, education, employment, and stable housing.
