GREEN BAY - Nominations are now being accepted for the Green Bay Packers Community Quarterback Award, presented by Humana, Inc.  Non-profit organizations are invited to nominate outstanding volunteers who demonstrate leadership, dedication, and a commitment to bettering their communities through volunteerism.

Two recipients in each of the following categories will be selected: volunteer leadership (i.e., board member), youth (age 13-17), adult (age 18-65), senior (over the age of 65), and small group (five or fewer volunteers).  Recipients will receive $4000 for the charity that nominated them, in addition to two tickets (provided by Humana) for the December 28, 2014 contest against the Detroit Lions at Lambeau Field.

All nominations must be submitted online by the end of the day Tuesday, October 14, 2014.  Check out the website for application and additional information:
